All of our physicians and midlevel providers are accepting new patients. Newborns being seen for the first time: Transferring from another practice? Older children transferring in are best served by making an appointment with the provider after we receive records from your previous physician. Transfer of records forms are available at the bottom of this page. We accept most insurance plans - please call our business office (631-499-4114) or check your insurance list of providers if you have questions. We are OPEN 7 days a week to serve you & 365 days a year! There is always an on-call provider to handle urgent needs after hours. Specific hours are located on our Hours & Location Page.
